From time to time the Practice may have to contact you regarding appointments, test results and so on. It is therefore important that the contact information we hold for you is accurate.

Can you please ensure that you inform us of any changes to your address or telephone contact numbers. This is particularly important if you change mobile telephone contracts and numbers.
